It's actually three changes. The first is the use of Optifine, which drastically improves performance (as well as adds a lot of neat additions). The shaders mod adds the shade from the everything and gives it a more 'realistic feel'. The last is a 256 pixel texture pack which makes the things look 'real'. The texture pack also makes the super ugly toolbar.
I will say you will need a pretty powerful computer to handle this. I've removed my overclocking, but a 3.3ghz i5-2500k cpu and a 7850 video card with 8gb of RAM was running this at 40 fps. Surprisingly, this is a much lower rate than Skyrim with over 8gb of downloaded texture enhancements. (not that '8gb' means anything, just that there are a shit ton of improvements).
Also, my minecraft server is still up and running if anybody wants to join in!
